The Tax Questionnaire and New Client Checklist are important forms that will help us collect and report necessary information in preparing your tax returns.
All new and existing clients must fill out the Tax Questionnaire. All new and existing clients should utilize the New Client Checklist to determine what documents to provide us with.
Please take the time in completing these forms, it could save you money in penalties and interest.
Once you have completed the Tax Questionnaire and New Client Checklist, upload these documents along with your 2023 tax documents to SmartVault.
